What do I do about my vagina and anal skin being sore and bleeding?

I don't know how it happened, it went away but now it's happening again. The skin right outside and a little inside my vagina hurts and bleeds a little. It hurts when I pass stool, when I pee, and when I have sex. When I pee or pass stool I have to lean forward a little so it won't hurt. Is it an underwear rash? a cleanliness issue? please help!! It's really awkward going to the doctor about this :((
